Macabre Marionette is a temporary companion obtained once per year during the Day of the Dead world event by completing the quest The Grateful Dead for your race's Cheerful Spirit.

Log In During Event
The Macabre Marionette is only obtainable during the Day of the Dead world event, so log in while the event is active.
Gather Required Ingredients
Purchase Ice Cold Milk from any innkeeper (not in Dalaran) and Simple Flour from a cooking supplier — both are needed to craft the Bread of the Dead.
Travel To Your Graveyard
Visit the graveyard associated with your race's home city: Humans go to Stormwind, Orcs/Goblins/Trolls go to Durotar, Blood Elves go to Eversong Woods, Undead go to Undercity, Tauren go to Mulgore, Dwarves/Gnomes go to Dun Morogh, Draenei go to Azuremyst Isle, and Night Elves/Worgen go to Darnassus. Pandaren and players without a racial graveyard can use Dalaran or Shattrath instead.
Buy Marigold And Recipe
At the seasonal vendor near the graveyard, purchase an 
Orange Marigold and the Recipe: Bread of the Dead, then learn the recipe.
Cook Bread Of The Dead
Use the cooking fire located beside the quest giver to craft the Bread of the Dead with your milk and flour.
Reveal And Talk To Spirit
Use the Orange Marigold to make the nearby spirits visible — you have only 30 seconds per flower use — then quickly accept the quest The Grateful Dead from the Cheerful Spirit that appears.
Turn In Quest Immediately
Hand in the Bread of the Dead to complete The Grateful Dead while the spirit is still visible; the Macabre Marionette is your reward.
Tips
If you have no racial graveyard or are playing a Pandaren, head to Dalaran where the Cheerful Dalaran Spirit offers the quest to players of any race — see the video guide for a visual walkthrough of this location.
