Complete Chapter 4 Campaign
Finish the fourth chapter of The War Within campaign, ending with Sour Victory, on the character you want to receive Spotty. Completing this chapter on an alt does not unlock the questline for other characters.
Find Humphrey Shortswig
Travel to Undermine and find Humphrey Shortswig at 38.19, 88.47, who will offer you the first quest of the Oh, Rats! optional storyline.
Complete the Storyline Quests
Work through all five quests of the Oh, Rats! storyline. The final quest, Unsolicited Feedback, has a note location at 32.46, 55.92 in Undermine, and completing it rewards Spotty.
