When you enter the cave, walk up the left side, as you can't interact with the object from the bottom floor.

Willie) is a companion pet dropped from a treasure chest hidden inside a cave in the Voidstorm zone, and obtaining it counts toward the Treasures of Voidstorm achievement.

Head to Cave Entrance
Travel to the cave entrance in Voidstorm at 38.06, 68.74. This is where you'll enter to find the treasure containing Willie.
Navigate Up the Left Side
Once inside the cave, move up along the left side — you cannot interact with the treasure object from the bottom floor, so staying to the left is required to reach it.
Loot the Half-Digested Viscera
Find and loot the Half-Digested Viscera treasure at 38.04, 68.83 or 37.69, 69.75 inside the cave to receive Willie. This treasure is part of the Treasures of Voidstorm achievement.
Tips
Willie has unique idle behaviors: he sniffs the ground and air, and his long fluffy ears move independently when he walks or runs. If you target him and jump or /cheer, he will track your movements and look directly at you.
