Enter the Forbidden Library
Head to the Forbidden Library in Revendreth at 37.65, 68.78. Once inside, locate the small 
Forbidden Ink bottle sitting on the left table at 37.68, 68.74 and loot it.
Exit and Reach the Wall
Exit the library and turn South-East toward the wall above at 38.4, 69.25. Climb the rocks and hop onto the building cornice, hugging the wall as you move forward toward the ledge entry point at 38.66, 68.14.
Drop Down to Lost Quill
Drop down to the next ledge where the Lost Quill is located at 37.74, 69.26, on the archway where trees grow out of the mountain. The quill cannot be seen from below, so follow the wall carefully until you reach the exact spot at 37.76, 69.27.
Return the Ink and Loot
Talk to the Lost Quill and use the dialogue option that appears because you have the Forbidden Ink in your bags. The quill will reward you with the Refilling Inkwell, which contains the Lost Quill battle pet.
Tips
If you cannot climb up to the ledge using the rocks alone, using Door of Shadows as a Venthyr ability makes reaching the Lost Quill at 37.74, 69.26 significantly easier — see the video guide for an alternative route if you do not have that ability.
The Lost Quill treasure is part of the Treasures of Revendreth achievement, so looting it also counts toward that criteria.
