Guide awaiting review
Start the Naga Attack! World Quest
Wait for the 'Naga Attack!' world quest to become available — it can appear in zones such as Zuldazar. Kill and loot Naga enemies during this world quest until you obtain the Azsharan Medallion, which automatically begins the quest chain.
Complete the Quest Chain
Work through the following quests in order: 'The Azsharan Medallion', 'Orders from Azshara', 'Every Little Death Helps', 'Unintended Consequences', 'The Pirate's Treasure', 'The Tempest Crown', 'Twist the Knife', and finally 'His Eye Upon You'.
Cleanse N'Zoth's Gift
At the end of the chain, you will be given a choice: cleanse N'Zoth's gift or keep it. You must choose to cleanse the gift to receive the Twitching Eyeball toy as a reward — keeping the gift does not award the toy.
Consider Using an Alt
If you want both outcomes, cleanse the gift on an alternate character to obtain the toy, and keep the gift on your main character, as Blizzard has indicated the gift may have future in-game purposes.
Tips
See the video guide for a full walkthrough of the entire quest chain from start to finish.
