The 
Personal Ball and Chain is a toy with a chance to drop from Zovaal's Vault in The Maw during the Shadowlands-era daily quest Full of Surprises.

Enter the Shadow Maw
Use a 
Collapsing Riftstone to enter the shadow version of The Maw. If you are the last person to use the portal, all other players in your group will be able to enter as well — enter last to avoid leaving anyone behind.
Accept the Daily Quest
Once inside the shadow version of The Maw, accept the daily quest Full of Surprises from Ve'Nari at 43.5, 50.9. Accept the quest before interacting with the chest, and note that the quest can be shared with your group.
Locate and Drag the Chest
The vault chest can spawn at four different positions in The Maw — check at 47.2, 79.7, at 62.1, 64.3, at 66.4, 58.2, and at 33.0, 66.3. Drag the chest back to Ve'Nari, which takes approximately 5 to 7 minutes. Groups larger than 5 players can help drag the chest, but the group must not be in raid formation.
Open the Vault
Turn in the Full of Surprises daily to open Zovaal's Vault. The Personal Ball and Chain has a chance to be inside. The vault can contain 1 or sometimes 2 Collapsing Riftstones in addition to any other loot.
Tips
The Personal Ball and Chain grants a 5-minute buff that slows you by 45% and has a 5-minute cooldown. It survives blinks but breaks on teleports such as those found in Oribos.
