The 
Hungry Black Hole is a toy dropped from the Bloody Sack in the Voidstorm zone, obtained by collecting nearby meat and throwing it into the Forgotten Oubliette until it spits out the sack.

Travel to Forgotten Oubliette
Head to the Forgotten Oubliette in the Voidstorm zone at 64.42, 75.51. This is where the Bloody Sack can be obtained.
Collect and Throw Meat
Collect the meat scattered around the area near the Forgotten Oubliette at 64.5, 75.49, then throw it into the Forgotten Oubliette. Keep throwing meat in until the Forgotten Oubliette spits out the Bloody Sack.
Loot the Bloody Sack
Open the Bloody Sack to receive the Hungry Black Hole toy.
Play the Black Hole Minigame
Using the toy places you into a minigame where you control a Black Hole with your arrow keys. Catch the falling meat to score points while avoiding falling stars, which deal damage. You start with three lives.
Tips
The minigame tracks a high score, so try to catch as much falling meat as possible while steering clear of the stars.
