Drops from Yiphrim the Will Ravager. You can farm it in Raid group and click the crystal as often as you want! You do NOT need to kill the adds because the rars can spawn without killing any add, i see it very often.

Wait for Uldum Assault
Yiphrim the Will Ravager only appears during an active Black Empire assault in Uldum. The assault must be up before any of the following steps will work.
Locate the Summoning Portal
Head to the Neferset area in Uldum and find an active Summoning Ritual portal event. The portal is not up every day and takes roughly 2 minutes to respawn after being used.
Complete the Summoning Ritual
Kill all the Acolyte of N'Zoth at the portal event location and then close the summoning portal. You can only receive quest credit for closing the portal once per day, but other players closing the portal also counts toward triggering the rare spawns.
Repeat Portal Clicks in a Raid
Form a two-player raid group and have players repeatedly click the summoning portal each time it becomes available. Testing has shown that the same player can click the portal multiple times within a raid group. After enough portal completions, a set of three rares will spawn around Neferset.
Kill Yiphrim the Will Ravager
Once the rare set spawns, check all three spawn points around Neferset. Yiphrim the Will Ravager can appear at 50, 78.4 in Uldum — kill him for a chance at the Budget K'thir Disguise. The other two spawn points are located at 50, 88 and 55, 79.
Tips
After one set of three rares is killed, repeated portal clicks by a raid group can trigger a second set of rare spawns — one group recorded roughly 15 portal interactions between the first and second rare spawns, approximately 30 minutes apart.
Yiphrim the Will Ravager is only one of six possible rares that can appear at the three spawn points, so he is not guaranteed to be present in every rare set that spawns.
