Helm of Raging Tempests is crafted by upgrading the Primalist Plate Helm drop from The Forbidden Reach rares to level 2/3 (ilevel 385), then optionally to 3/3 (ilevel 395).
Farm Primalist Plate Helm
Hunt rare enemies in The Forbidden Reach until Primalist Plate Helm drops. This item is the base material needed to create the transmog.
Upgrade to level 2/3
Use the Primalist Plate Helm to craft Helm of Raging Tempests at upgrade level 2/3, reaching ilevel 385.
Optional: Upgrade to 3/3
Upgrade the helm to level 3/3 (ilevel 395) by obtaining 
Untapped Forbidden Knowledge from rares in The Forbidden Reach.
Tips
The Primalist Plate Helm drop rate is extremely low; expect to kill well over 100 rares before obtaining it.
