Serpentstalker Guise is a transmog item obtainable from the weekly Timewalking quest cache that rewards Antorus raid gear.
Complete Weekly Timewalking Quest
Run the A Shattered Path Through Time weekly Timewalking quest on a character that has not cleared any bosses in Antorus, the Burning Throne raid. Upon completion, you will receive a cache containing a random piece of 930+ item level gear from Antorus.
Open Raid Cache
Open the cache reward to receive your Antorus item. The gear can be warforged or titanforged, scaling up to 985 item level.
Tips
If you have completed at least 12 boss kills in Antorus Normal difficulty before starting the quest, the cache will reward Normal difficulty gear instead of Raid Finder level.
If you have completed at least 12 boss kills in Antorus Heroic difficulty before starting the quest, the cache will reward Heroic difficulty gear.
