Enter Tomb of Sargeras on Mythic difficulty and kill trash mobs throughout the raid. The belt drops from trash enemies like Eternal Soulgard mobs encountered on the way to bosses such as Desolate Host. Mythic difficulty has confirmed drops as of April 2025.
Verify the correct item level tint
When examining the belt on the Auction House, confirm you are purchasing the Mythic item level (247 ilvl) version to ensure you receive the correct mythic-colored appearance, as lower item level versions may display with a generic color in the listing.
Tips
