Vol'jin's Spear is a Horde faction tied to Ashran in Warlords of Draenor, and reaching Exalted rewards both the 'Hero of Warspear' achievement (under PvP → Ashran) and the 'Vol'jin's Spear' achievement (under Reputation → Draenor), along with the title '%, Spear of Vol'jin'.
Earn Rep in Ashran
Participate in the Epic Battlefield version of Ashran to earn reputation. Turning in 5 artifacts awards 25 reputation per turn-in. Note that Ashran is no longer listed under Warlords of Draenor content but instead appears under Horde Forces alongside other PvP factions.
Loot PvP Insignias
Looting PvP insignias from enemy players in Ashran is a faster method of gaining reputation than artifact turn-ins, though as of patch 9.1.5 these insignias stopped dropping — artifact turn-ins remain the confirmed working method until this is resolved.
Avoid Medallion of the Legion
The 
Medallion of the Legion does not grant reputation with Vol'jin's Spear, even though it works for other Warlords of Draenor factions.
Visit Vendor Dazzerian
Once you have accumulated enough reputation, visit Dazzerian at 48.4, 23.8 to browse and purchase faction rewards.
Tips
Artifact turn-ins in Ashran are the most reliable source of Vol'jin's Spear reputation currently available, as PvP insignia drops from enemy players have been reported as non-functional since patch 9.1.5.
