Hellscream's Tome of Destruction (Heroic)
The 
Hellscream's Tome of Destruction is a Heroic-difficulty Heirloom offhand for Caster DPS, obtained by defeating Garrosh Hellscream in the Siege of Orgrimmar raid on Heroic difficulty, and scales from Level 90 to Level 100.
Enter Siege of Orgrimmar
Set the Siege of Orgrimmar raid to Heroic difficulty and enter the instance. Fight through the raid until you reach Garrosh Hellscream, the final boss.
Defeat Garrosh Hellscream
Defeat Garrosh Hellscream on Heroic difficulty. As of Patch 6.0, players who have not yet received any Heirloom from Garrosh are guaranteed a spec-appropriate Heirloom drop on their first kill on Normal difficulty or higher, with increased chances for additional Heirlooms beyond the first.
Note on Caster Main-Hand
There is no Caster main-hand Heirloom available from Garrosh, so pairing this offhand with the 
Hellscream's War Staff instead is worth considering, as it will scale to Level 100 unlike non-Heirloom alternatives such as 
Rik'kal's Bloody Scalpel or 
Arcweaver Spell Sword, which cap at a fixed item level.
Tips
All Hellscream Heirloom weapons — across Normal, Heroic, and Mythic difficulties — ultimately scale to a maximum item level of 633, with Heroic versions increasing approximately 7 item levels per character level gained.
