Hellscream's Cleaver is a Heroic-difficulty Heirloom one-handed axe obtained by defeating Garrosh Hellscream in the Siege of Orgrimmar raid.
Enter Siege of Orgrimmar
Set the raid to Heroic difficulty, then enter Siege of Orgrimmar and make your way to Garrosh Hellscream, the final boss of the instance.
Defeat Garrosh Hellscream
Defeat Garrosh Hellscream on Heroic difficulty; as of Patch 6.0, players who have not yet received any Heirloom from this boss are guaranteed a spec-appropriate Heirloom drop on their first kill, with increased chances for additional Heirlooms beyond the first.
Collect Hellscream's Cleaver
Hellscream's Cleaver scales up to item level 633 as you level from 90 to 100, with the Heroic version gaining approximately 7 item levels per step along the scaling curve.
