Show off your sparkling smile by using a Tooth Pick.
That Sparkling Smile is a 10-point Hallow's End achievement earned by using a 
Tooth Pick, which applies a 1-minute Sparkling Smile buff that makes your teeth gleam even through your helmet.
Obtain a Tooth Pick
Purchase a Tooth Pick from Dorothy or Chub for 2 
Tricky Treats each, or open a 
Handful of Treats obtained from candy buckets found in inns during Hallow's End. Alternatively, trick-or-treat an innkeeper by selecting the option in their conversation window — this can only be done once per hour due to the Tricked or Treated debuff, and may award Tooth Picks.
Use the Tooth Pick
Right-click your Tooth Pick to use it; this instantly completes the achievement and applies the 1-minute Sparkling Smile buff.
Tips
Tooth Picks come in a soulbound stack of ten, so a single purchase or drop gives you multiple uses.
