Avast Ye, Admiral! requires you to obtain the Bloodsail Admiral's Hat by grinding reputation with the Bloodsail Buccaneers, rewarding you with the Bloodsail Admiral title.
Start Quests in Stranglethorn
Talk to Sea Wolf MacKinley and complete his two quests, Scaring Shaky and Kill-Collect, which will unlock the Bloodsail Buccaneers quest from First Mate Crazz.
Grind Bloodsail Reputation
Kill Booty Bay Bruisers to earn Bloodsail Buccaneers reputation — be aware this requires killing over 1,000 Bruisers, and your reputation will cap at 11999/12000 Honored.
Tips
If you completed Stranglethorn Vale quests before Cataclysm and had them reset, you can restart the chain from Sea Wolf MacKinley for a smoother grind.
