Head Case
Defeat Megaera in Throne of Thunder on Normal or Heroic difficulty without cutting off a head of the following types:
Head Case requires defeating Megaera in Throne of Thunder without cutting off a head of each color type (Red, Blue, and Green) — one color per week across three separate weekly lockouts.
Understand the Multi-Week Structure
This achievement takes a minimum of 3 weekly resets to complete. Each week, you leave one color of head (Red, Blue, or Green) completely untouched while killing the other two colors. Rotate which color you protect each week until all three have been spared across separate lockouts.
Enter Throne of Thunder
Enter Throne of Thunder and begin the Megaera encounter. Designate one head color to never attack this week — communicate this clearly to anyone in your group before the pull.
Use Only Single-Target Attacks
Attack only the heads of the two permitted colors and use strictly single-target abilities. Avoid multishot, area-of-effect abilities, or any buffs that deal damage to unintended targets, as accidental hits on the protected color head will count against you.
Know the Elemental Blood Stacks
By skipping one color entirely, the Elemental Blood of Megaera stacks for the remaining two colors will build faster, increasing their damage output and the rate of special abilities like cinders, ice beams, and explosions — so expect the fight to hit harder than a standard kill.
Repeat for Two More Lockouts
Return the following week and protect a different color, then again the week after for the final color. After three separate weekly kills — each with a different color head left alive — the achievement is complete.
Tips
On Heroic difficulty, do not alt-tab mid-fight — the Nether Wyrm adds will perma-stun you with no way to recover, requiring a force-close and waiting for a soft reset.
The popular Heroic strategy is to leave Green heads uncut, as this is generally considered the most manageable color to skip.
