And Stay Dead!
Allow High Inquisitor Whitemane to cast Mass Resurrection, defeat all revived Scarlet Judicators, and then defeat Whitemane in Scarlet Monastery on Heroic Difficulty.
And Stay Dead! is a 10-point Heroic difficulty achievement in Scarlet Monastery that requires you to let High Inquisitor Whitemane cast Mass Resurrection, kill all revived Scarlet Judicators, and then defeat Whitemane.
Clear the Left Wing
Before engaging any bosses, clear all Scarlet Judicators in the left (East) wing of the Cathedral. Whitemane needs more than 13 Judicator corpses present in that wing to cast Mass Resurrection.
Kill Commander Durand
Defeat Commander Durand. High Inquisitor Whitemane will then emerge and begin casting Mass Resurrection after a few seconds.
Let Whitemane Cast Resurrection
Do NOT interrupt Mass Resurrection — it has a long cast time and is interruptable, so simply allow it to complete. Whitemane will revive all 13 Scarlet Judicator corpses from the left wing. At max level, you do not need to damage Whitemane at all to trigger the cast.
Defeat All Scarlet Judicators
Burst down all revived Scarlet Judicators. Single-target them to avoid accidentally killing Whitemane before all Judicators are dead. The 
Shado-Pan Dragon Gun is effective here as its damage does not decrease with additional targets.
Defeat High Inquisitor Whitemane
Once all Judicators are dead, kill High Inquisitor Whitemane (and Commander Durand again if he was resurrected) to complete the achievement.

At max level, skipping damage on Whitemane entirely and going straight to killing the Judicators after Mass Resurrection prevents accidentally one-shotting the boss before the achievement triggers.
