Realm First! Level 90 (Legacy) was awarded to the first player on each realm to reach level 90 during the Mists of Pandaria expansion, before Shadowlands launched.
Reach Level 90 First
This achievement required being the first player on your realm to reach level 90 from level 85. The world first was claimed by a monk player named Алангасар on Lich King realm approximately 6 hours after launch, while EU world first went to Fs on Ravencrest, achieved before US servers had even opened.
