Mystically Epic
Equip an epic item in every slot with a minimum item level of 476 (prior to Battle for Azeroth).
Mystically Epic requires you to equip an epic item of item level 476 or higher in every gear slot, using pre-Battle for Azeroth items.
Use Conquest Points Method
Visit the Conquest quartermaster and buy a Conquest item, equip it, unequip it, and return it for a full refund, then repeat this process for each gear slot — including equipping rings and trinkets in both of their available slots.
Gear Head, Shoulders, Chest, Belt, Legs
Head, Shoulders, Chest, Belt, and Legs are available as epic rewards from various faction vendors at Revered reputation — Chest also has a crafted option available.
Gear Wrists, Gloves, Rings
Wrists, Rings, and Gloves can be obtained from faction vendors at Honored reputation, and Gloves also have a crafted option available.
Gear Neck and Trinkets
An epic Necklace is available from a faction vendor at Exalted, or alternatively from a vendor for Valor Points at Honored; Trinkets are available from faction vendors at Revered, and Darkmoon Faire also offers epic trinkets — only a single 476+ trinket is needed since equipping it in one slot satisfies both trinket slots.
Gear Boots from Sha of Anger
Epic boots in the form of 
Claw of Anger drop from Sha of Anger on your first kill, so enter the encounter, defeat Sha of Anger, and loot the body.
Gear Weapons via Drops or Crafting
Some Heroic dungeons drop epic weapons, such as 
Headmaster's Will from Darkmaster Gandling, and crafted options include 
Inscribed Serpent Staff from Inscription.
Tips
Upgraded items do not count toward this achievement — for example, an upgraded Malevolent weapon that reaches 478 through upgrades will not satisfy the requirement.
Coren Direbrew trinkets are item level 470 and do not count toward this achievement, so avoid relying on them.
