Don't Stand So Close to Me
Defeat Morchok in Dragon Soul on Normal or Heroic Difficulty without any two players (three players on 25-player mode) ever being within 5 yards of each other while Morchok is channeling Black Blood of the Earth.

Burn Morchok Before Channeling
Enter Dragon Soul and use Heroism/Bloodlust to burn Morchok down before he begins channeling Black Blood of the Earth — if he dies before reaching that phase, the achievement is awarded automatically.
Spread Out During Black Blood
If Morchok does reach the Black Blood of the Earth channel, every player must be more than 5 yards from all other players for the duration — after he pulls everyone in during Falling Fragments, rotate your camera and be ready to move the instant the immobilization ends.
Track Achievement Progress In-Game
Shift-click the achievement in your achievement UI during the encounter — it will turn red immediately if the requirement is failed, giving you a reliable in-game way to confirm whether the attempt is still valid.
