Complete the Collecting Remains Quest in Midnight 5 times.
"Remains in the Rise" requires you to complete the Collecting Remains quest in Midnight 5 times by gathering Void-Tainted Remains and turning them in.
Collect Void-Tainted Remains
Fly around Slayer's Rise in Voidstorm and look for small skulls marking the remains. Looting these will award you between 2–5 
Void-Tainted Remains each.
Turn in remains to Deminos
Turn in your collected Void-Tainted Remains to Deminos Darktrance at 39, 82 to complete the quest.
Repeat 5 times
Complete the quest cycle 4 more times to earn the achievement.
Tips
The achievement can be completed without enabling War Mode.
