Phase-Lost-and-Found
Collect all of the Phase-Lost item appearances found from Phase Diving orbs.
Phase-Lost-and-Found requires collecting all 28 weapon appearances from Phase Diving orbs scattered across K'aresh and Tazavesh, ultimately rewarding the 
Phase-Lost Slateback mount.
Unlock Rank 3 Reshii Wraps
Before any orbs are visible to you, your 
Reshii Wraps must be at rank 3 with Secrets of the Depths unlocked, meaning 6 total strands. Without this requirement met, no Phase Diving orbs will appear for you.
Collect 28 Weapon Appearances
Each orb has approximately a 20% chance to award a weapon appearance, and you will always receive one you have not yet learned. There are 28 total appearances to collect, including the 
Phase-Lost Claw, which does not appear in the achievement tracker but is required to complete it. The other appearances include the

Phase-Lost Sigil, 
Phase-Lost Beacon,

Farm Orbs in Zone 1 (K'aresh)
At least one orb is always active somewhere in the zone, and multiple can be up simultaneously, though two players cannot claim the same orb. In Zone 1, orbs spawn at 51.02, 69.12; 51.17, 67.74; 52.17, 64.88; 54.32, 63.17; 54.91, 63.81; 59.41, 60.44; 58.93, 57.51; 60.52, 55.49; 55.28, 55.45; and 47.8, 60.85. Additional K'aresh spawns are found near the raid entrance ramp at 43.23, 21.58, and across three Manaforge positions at 44.08, 16.89; 49.21, 18.55; and 49.19, 26.7. The Fracture of Laacuna area has orbs at 55.96, 21.54; 56.55, 20.92; 57.9, 23.93; and 59.83, 22.69. A cluster of five orbs sits at 50.11, 36.2; 50.57, 35.1; 50.95, 36.71; 48.6, 38.45; and 47.73, 37.28. One orb spawns on Shadow Point on a rock at 47.17, 15.79.
Farm Orbs in Tazavesh
Inside Tazavesh (map ID 2472), orbs spawn in front of the portal at 53.17, 60.48; next to the edge at 44.15, 34.28; on the bridge at 44.24, 27.1; at the top of the stairs at 42.74, 49.93; and in the hallway at 34.97, 57.92. An additional Tazavesh-area orb is located at 66.86, 89.67.
Farm the Northern Small Island
The small island in the far north of the K'aresh map at 72.95, 10.94 is a strong farming location because its two Phase conduits are not connected to the rest of the conduit network and do not allow teleportation. This means the only way to reach the island is by flying, which significantly reduces competition from other players. Two orbs spawn within sight of each other near 73.1, 11.0, and you can watch both from a central position between them.
Use Additional Spawn Locations
Further orb positions include 43.77, 25.58; 48.91, 26.71; 62.58, 24.07; 60.35, 28.41; 60.6, 29.66; 70.33, 31.99; 73.98, 57.49; 50.49, 54.12; and 68.77, 47.77 in K'aresh. 
Untethered Xy'bucha orbs are also found at 49.67, 17.05; 49.65, 26.65; 50.74, 58.45; 53.75, 45.37; 54.81, 48.18; 61.33, 39.11; and 80.35, 48.69. A server-hop-friendly cluster is at 45.74, 51.55, where collecting from a world boss group finder can yield a high volume of orbs in a short session.
Tips
The
Phase-Lost Claw is not shown in the achievement tracker, so if you complete all listed appearances but the achievement does not complete, continue collecting orbs until the hidden appearance is awarded.The Overlook, Tazavesh, and north of Shan'dorah areas have relatively frequent orb respawns and are worth prioritizing in a rotation.
The northern small island at 72.95, 10.94 has reduced player competition because its conduits cannot be used for teleportation, making it only accessible by flying.
Two orbs can be awarded simultaneously from a single session, though this is rare.
