Allow three Freezing Vapors to cast Coalesce during the Admiral Ripsnarl encounter in Deadmines on Heroic Difficulty.
It's Frost Damage requires you to allow three Freezing Vapors to complete their Coalesce cast during the Admiral Ripsnarl encounter in Deadmines on Heroic difficulty.
Enter Deadmines on Heroic
Enter the Deadmines on Heroic difficulty and engage Admiral Ripsnarl. Avoid using high-damaging abilities when he is low on health, as it is possible to kill him before the vapor phase triggers — stop attacking once he vanishes and the Freezing Vapors appear.
Control Ripsnarl's Health
Bring Ripsnarl below 75% health to trigger the fog phase where Freezing Vapors spawn, then stop dealing damage to him entirely. At max level, unequipping your gear before the fight is a reliable way to avoid accidentally killing him before all three Coalesce casts occur. 
Whole-Body Shrinka' also works well for reducing your damage output.
Let One Vapor Coalesce
Kill other Freezing Vapors as normal but designate one to leave alive. Tank that vapor to the side of the ship away from the door to the central cabin. Once it begins casting Coalesce, retreat inside the cabin to break line of sight and avoid the explosion and knockback. The Evolving Vapor does not move while casting, so this is consistently avoidable.
Repeat Two More Times
Return outside after each Coalesce cast completes, pick the next Freezing Vapor, and repeat the process two more times. You do not need to kill Admiral Ripsnarl to earn the achievement — it completes after the third Coalesce cast lands.
Tips
If you accidentally kill the vapors too quickly, you can let Ripsnarl reset and re-engage without killing him to trigger the vapor phase again.
Unequipping all gear before the pull is the most reliable solo method to prevent one-shotting the Freezing Vapors or killing Ripsnarl too fast.
