Rotten to the Core requires you to defeat 60 Disciples within 10 seconds during the High Priestess Azil encounter in the Stonecore on Heroic difficulty.
Engage High Priestess Azil First
Enter the Stonecore on Heroic difficulty and pull High Priestess Azil before touching any of the Disciples. You must be in active combat with the boss for kills to count toward the achievement.
Pull All Three Disciple Groups
There are three groups of 20 Disciples located directly in front of High Priestess Azil — pull all three groups together while remaining in combat with the boss.
Kill One Disciple to Start Timer
Kill exactly one Disciple to start the 10-second timer, then immediately AoE down the remaining 59 Disciples before the timer expires to complete the achievement.
