"And I'll Form the Head!" is an achievement earned by obtaining 
Mimiron's Head from 25-player Yogg-Saron with zero Keepers assisting you, a condition known as Yogg+0.
Enter Ulduar, Skip All Keepers
Enter Ulduar and progress to Yogg-Saron without activating any of the four Keepers as assistance — all four must be denied before the encounter begins.
Defeat Yogg-Saron on 25-Player
Defeat Yogg-Saron on 25-player difficulty with no Keepers assisting; the fight is two-tankable and the full Ulduar clear to this point takes approximately two hours.
Loot Mimiron's Head
Mimiron's Head drops at a 100% rate currently, but Blizzard has stated it will become a rare drop in a future patch, so farm it before that change takes effect.
Tips
Because the drop rate is currently 100%, this is farmable on a weekly lockout reset until all desired characters have obtained it.
