Enter Icecrown Citadel

Enter Icecrown Citadel on 25-player mode and make your way to the Lich King encounter on the Frozen Throne.

Wait for Drudge Ghouls

Stand on the stairs and wait for the Lich King to summon at least 9 Drudge Ghouls — these adds are essential for allowing Necrotic Plague to jump and accumulate stacks.

Trigger Necrotic Plague

Summon a helper mob or use a pet to attack the Lich King so he casts Necrotic Plague on you, then let it expire off of you and jump between the adds.

Let Plague Stack to 30

Tab through the adds to monitor Necrotic Plague stacks; wait until at least one add reaches 30 stacks — waiting until 34 stacks is a safe buffer to ensure the achievement registers.

Kill the Lich King

Once Necrotic Plague has reached 30 or more stacks, burst the Lich King down immediately and wait through his death cinematic for the achievement to fire.

Tips

  • Necrotic Plague was broken in patch 8.1.5 but was confirmed fixed in patch 8.3.7, so if you attempted this during that window and failed, it is now working correctly.

  • If you are playing a Hunter, set your pet to passive and Stay near the Lich King, use Misdirect to transfer aggro to it, then Feign Death away — Necrotic Plague does not jump to the pet, making it a safe anchor.

  • Post-Shadowlands, your damage output or your helper mob's damage may push the Lich King into phase 2 prematurely, so use the weakest possible helper and avoid hitting the Lich King directly until the plague has reached 30 stacks.
