Allow Necrotic Plague to stack to 30 before defeating the Lich King in 10-player mode.
Been Waiting a Long Time for This (10 player) requires you to let Necrotic Plague stack to 30 before killing the Lich King in Icecrown Citadel on 10-player mode.
Understand Necrotic Plague Stacking
Necrotic Plague gains a stack each time it kills a target or expires — not each time it jumps between targets. The Lich King gains a debuff called Plague Siphon each time Necrotic Plague jumps, so do not confuse 30 stacks of Plague Siphon with 30 stacks of Necrotic Plague; watch the add's debuff, not the Lich King's.
Build Up Adds in Phase 1
During Phase 1, allow the Lich King to summon at least 9 Drudge Ghouls (or several Shambling Horrors if soloing) before getting Necrotic Plague onto them — delay this as long as possible without pushing the Lich King below 70% health, which would trigger Phase 2 prematurely.
Bounce Plague Across Adds
Once Necrotic Plague is on an add, let it jump repeatedly between the grouped adds — each time it kills an add or expires, the stack count increases by one; keep the adds alive and grouped so the plague has targets to bounce to until it reaches 30 stacks.
Burn the Lich King
Once Necrotic Plague has reached 30 stacks on an add, deal burst damage to the Lich King to defeat him and the achievement will be awarded.
Tips
As a Hunter, place your pet on passive near the Lich King and use Misdirection to generate threat, then Feign Death away from the boss and wait for enough adds to accumulate before engaging Necrotic Plague.
When running with two players, have one player stand at maximum range so fresh Necrotic Plague casts land on them rather than bouncing prematurely to already-infected adds, giving you more control over stack timing.
Necrotic Plague failed to stack in patch 8.1.5 but was confirmed fixed in patch 8.3.7, so this achievement is currently obtainable.
