The Sword in the Skull
Recover the missing hilt of Quel'Delar, draw the blade from its resting place, reforge the weapon, purify it in the Sunwell, and present it for your just reward.
The Sword in the Skull is a 10-point achievement earned by completing the Quel'Delar quest chain, which begins with the 
Battered Hilt dropped in the Heroic versions of The Forge of Souls, Pit of Saron, or Halls of Reflection in Icecrown.
Obtain the Battered Hilt
The Battered Hilt drops from all mobs in the Heroic versions of The Forge of Souls, Pit of Saron, and Halls of Reflection. Enter any of these three Icecrown dungeons on Heroic and clear all mobs to maximize your chances per run. The item is also available on the Auction House if you prefer to skip farming.
Complete the Quest Chain
Use the Battered Hilt to start the quest chain and follow it through all required steps, including recovering the missing hilt, drawing the blade, reforging the weapon, purifying it in the Sunwell, and presenting it for your reward. Completing all steps awards the achievement.
Tips
The achievement is found under Expansion Features, Argent Tournament — not under World Events.
