Powerful Concoctions IV requires you to craft elixirs, flasks, or phials 1,500 times using the Alchemy profession, and is the final achievement in the Powerful Concoctions series introduced in patch 10.1.7.
Stock Up on Herbs
Purchase Pandaria herbs from the Auction House — specifically 
Rain Poppy, 
Fool's Cap, 
Green Tea Leaf, 
Silkweed, and 
Snow Lily — along with 
Crystal Vials. Buying a variety of herbs rather than a single type helps work around limited Auction House stock.
Craft Pandaria Elixirs Repeatedly
Use your Alchemy profession to craft any combination of Pandaria elixirs such as Elixir of Peace (Rain Poppy x1, Fool's Cap x1, Crystal Vial x1), Mantid Elixir (Green Tea Leaf x2, Crystal Vial x1), Mad Hozen Elixir (Rain Poppy x2, Crystal Vial x1), Elixir of Weaponry (Silkweed x2, Crystal Vial x1), Elixir of Mirrors (Silkweed x1, Fool's Cap x1, Crystal Vial x1), Monk's Elixir (Rain Poppy x1, Silkweed x1, Crystal Vial x1), Elixir of the Rapids (Rain Poppy x1, Snow Lily x1, Crystal Vial x1), and Elixir of Perfection ( Silkweed x1, Snow Lily x1, Crystal Vial x1) until you reach 1,500 total crafts, which takes approximately 53 minutes.
Tips
Crafting 1,500 Elixirs of Peace back-to-back is a viable strategy if you can source enough Rain Poppy and Fool's Cap, and one player completed the full 1,500 crafts in approximately 53 minutes this way.
