Enter the Sanctum of Domination and pull the Remnant of Ner'zhul. After a few seconds, Orbs of Torment will spawn in the arena but have a 99% damage reduction, making them unable to be picked up immediately.

Before or after pulling, look to the left side of the arena for a circlet-like hoop hanging near a hanging cage. Once the boss is pulled, this hoop will begin swinging like a pendulum. There is also a second swinging ring you must align with the first — both must be lined up for a goal to count.

When the boss casts Suffering on you, move near the Orbs of Torment to weaken them. Once weakened, kill the orb so it becomes grabbable, then pick it up. You can use any movement-impairing removal effect to negate the slow applied while carrying the orb.

Carry the Orb of Torment to the edge of the platform closest to the goal hoop, then use the Extra Action Button to throw it. Wait for the apex of the pendulum swing and count to two before throwing for the best timing. A goal only counts when the hoop flashes and the chat message 'A goal has been scored' appears — do not rely on the visual alone.

Repeat the Suffering-weaken-grab-throw process two more times until three goals have been scored, then defeat the Remnant of Ner'zhul to complete the achievement.
