I Only Have Eyes For You
Defeat Duraka the Unbreakable in the Sanguine Depths on Mythic difficulty.
I Only Have Eyes For You is a 10-point achievement earned by finding two gem eyes and placing them into Duraka the Unbreakable to turn him hostile, then defeating him in the Sanguine Depths on Mythic difficulty.
Clear to Duraka's Room
Run the Sanguine Depths on Mythic difficulty and progress until you reach Duraka the Unbreakable. He is friendly by default, so do not engage him yet.
Loot the Left Gem Eye
In the room adjacent to Duraka the Unbreakable, find the 
Left Gem Eye hidden behind a statue. Once looted, the gem lasts only 10 minutes in your inventory, so move quickly — do not loot it if your bags are full or it will disappear.
Loot the Right Gem Eye
Before reaching the final boss platform, locate a coffin in the room just before it and loot the 
Right Gem Eye. The same 10-minute timer applies once it is in your bags.
Give Both Gems to Duraka
Return to Duraka the Unbreakable and use each gem from your inventory while standing next to him. Placing a gem on him removes the 10-minute timer permanently, so you can hand them over one at a time without pressure once they are on him.
Defeat Duraka the Unbreakable
Once both gems are placed, Duraka the Unbreakable turns hostile. Stay inside his room for the entire fight — being outside the room when he dies will prevent the achievement from being awarded.

If you plan to collect both gems before handing them in, remember each has a 10-minute timer — loot the Right Gem Eye last since it is closer to Duraka's room on the return path.
The chains used to return up the dungeon disappear after the final boss is killed, so collect both gems and complete this achievement before defeating the last boss.
