Start in Oribos
Pick up the quest 'If You Want Peace...' from Tal-Inara at 60.4, 37.1 in Oribos, then speak with Pathscribe Roh-Avonavi who is standing nearby to be sent to Maldraxxus.
Kill Arena Monsters
Complete the storyline step 'To Die By the Sword' by killing 30 monsters in the arena.
