Defeat N'Zoth on Heroic
Enter Ny'alotha, the Waking City and defeat N'Zoth the Corruptor on Heroic difficulty or higher. The boss will drop an item that begins a quest granting 
Uncorrupted Voidwing.
Complete before tier ends
Obtain the achievement before the next raid tier releases, as it becomes unobtainable afterward.
