Equip All 5 Faceless Masks
Equip your 5 Faceless Masks before entering — recommended masks include 
Daredevil, 
Dark Imagination, Multitudes, Nemesis, and Vengeance. You can select masks from Revisited Visions, but save your choices by actually starting the Vision, then return to the Chamber of Heart and launch the old Vision.
Collect Hidden Stormwind Buffs
Before engaging enemies, collect the hidden buffs scattered across Stormwind: find Augustus Moulaine for 7% Versatility, the Angry Bear Rug Spirit for 10% Haste, the Minefield buff for 10% increased damage done, and the Neglected Guild Bank for 10% Critical Strike.
Skip the Toymaker
The Toymaker does not need to be killed to complete this achievement, so you can leave him alone and focus your sanity resources on the required objectives.
Complete All Objectives
Complete every objective within the Horrific Vision of Stormwind in a single run — all 5 masks must remain active for the entire visit for the achievement to register.
