Complete the Horrific Vision achievements listed below.
Through the Depths of Visions is a meta-achievement requiring you to complete a set of Horrific Vision achievements, rewarding the 
Black Serpent of N'Zoth mount upon completion.
Complete Horrific Vision Achievements
Work through all sub-achievements listed under Through the Depths of Visions by running Horrific Visions, including completing runs with all 5 original BFA masks active: Dark Imagination, Pained, Burned Bridge, Long Night, and Daredevil.
Activate Masks Across Versions
If you have unlocked newer masks from the 11.1.5 version of Horrific Visions (such as Multitudes, Nemesis, or Vengeance), you can activate those in the newer version and then run the original BFA version — the masks remain active even without visible altars, counting toward the 5-mask achievement.
Handle Valley of Spirits Bug
In the Orgrimmar Vision, the Valley of Spirits objective can fail to complete if the final boss dies too quickly — instead of killing him in one shot, use a weak ability and allow him to cast Hopelessness, then run back to your body before finishing him off to ensure the objective registers correctly.
Tips
This achievement is also a criteria for the Rank 4 cosmetic essences Unwavering Ward, Strength of the Warden, and Spark of Inspiration, so completing it has additional progression value beyond the mount.
