Horde players can pick up the starting quest item at 50.01, 31.8 in Stormsong Valley (Warfang Hold), while Alliance players can start at 59.49, 68.34 in Stormsong Valley (Brennadam). This quest sends you to Wayne the Ancestral at 78.55, 55.21 in Forgotten Cove, an island south of Shrine of the Storm, unlocking his quest chain.

To fill the bar efficiently, find 2 Foul Larva on the shore near 82.47, 53.61 and let them hit you with Grave Bite. You can also pull 7 mobs at 83, 54 — 3 humanoids from the cave at 83, 54, 2 larvas from the fountain at 84, 54, 1 spider from the road at 83, 53, and 1 spider from the hill at 83, 52. If Warbringer Yenajz is active, his Endless Abyss ability fills the bar quickly.

Each time the strain bar fills, you enter The Stage is Set and receive one of six possible effects in random order: Afraid, Lost Cause, Sadistic (permanent debuffs), Surging Confidence, Empowered, or Clarity (10-minute buffs), or Crushing Dread (3-second debuff). Refill the bar repeatedly until you have all three permanent debuffs — Afraid, Lost Cause, and Sadistic — active at the same time to earn the achievement.

Once you have the achievement, speak to Wayne the Ancestral and select 'I am troubled. Can you help me.' He will direct you to an idol at 76, 50. Head out the door and down the hill to your left, then click the Purifying Effigy near the idol to spawn a mob. Killing it removes all your debuffs.
