Power Ascended
Unlock every Artifact Trait for a single artifact weapon after empowering it.
Power Ascended requires completing the Broken Shore artifact empowerment questline (ending with 'A Gift from the Six') and then unlocking every trait on your artifact weapon, culminating in the first point of Concordance of the Legionfall.
Complete the Empowerment Questline
Start the questline on the Broken Shore and complete it through 'A Gift from the Six' for your specific specialization. Completing this quest for a specialization unlocks Concordance of the Legionfall for that weapon — without it, the achievement will not fire even if you have Concordance unlocked and a point spent.
Unlock All Artifact Traits
With the questline complete, unlock every artifact trait on your empowered weapon and place one point into Concordance of the Legionfall to trigger the Power Ascended achievement.
Purchase Your Class Reward
Once the achievement is earned, purchase your class-specific reward for 
1000 Order Resources: Death Knights can buy a Bloodbrood, Frostbrood, or Vilebrood Whelpling from Amal'thazad depending on spec; Demon Hunters can buy 
A Tiny Set of Warglaives from Falara Nightsong; Beast Mastery Hunters can buy 
Tome of the Hybrid Beast from Pan the Kind Hand; Marksmanship Hunters can buy 
Trust of a Dire Wolfhawk from Pan the Kind Hand; Survival Hunters can buy 
Trust of a Fierce Wolfhawk from Pan the Kind Hand; and Mages can buy 
Magical Saucer from Uthel'nay. Each specialization with a qualifying weapon requires its own completed questline to unlock its individual reward.
Tips
If you want rewards from multiple specializations, you must complete the 'A Gift from the Six' questline separately for each specialization's weapon.
