Merely a Set
Collect any class armor set from Serpentshrine Cavern and The Eye.
Merely a Set requires you to collect any complete Tier 5 class armor set by gathering five tokens from bosses in Serpentshrine Cavern and The Eye, plus two crafted or trash-dropped pieces that also count toward the set.
Identify Your Token Group
Tier 5 tokens are split into three groups: Champion (Paladin, Rogue, Shaman), Defender (Warrior, Priest, Druid), and Hero (Hunter, Mage, Warlock). Death Knights, Monks, and Demon Hunters cannot obtain this tier.
Farm the Five Token Bosses
Enter Serpentshrine Cavern and defeat Leotheras the Blind for 
Gloves of the Vanquished [Champion/Defender/Hero], Fathom-Lord Karathress for 
Leggings of the Vanquished [Champion/Defender/Hero], and Lady Vashj for 
Helm of the Vanquished [Champion/Defender/Hero]. Then enter The Eye and defeat Void Reaver for 
Pauldrons of the Vanquished [Champion/Defender/Hero], and Kael'thas Sunstrider for 
Chestguard of the Vanquished [Champion/Defender/Hero].
Turn In Tokens to Vendor
As of patch 7.2, clicking a token automatically creates the appropriate item for your loot specialization without visiting a vendor. If you prefer the vendor route for PvE pieces, bring your tokens to Kelara (Aldor) or Veynna Dawnstar (Scryer) in Shattrath City.
Collect the Two Additional Pieces
The achievement also requires the 
Boots of Courage Unending, which drops from trash mobs inside Serpentshrine Cavern or The Eye — loot all trash — and the 
Belt of the Guardian, which requires a Blacksmith to obtain the recipe and craft the belt.
Tips
Both Serpentshrine Cavern and The Eye share a weekly lockout, so you can farm all five token bosses each week across both raids.
The Boots of Courage Unending drop from trash mobs, not bosses — loot every trash enemy in both raids to avoid missing this required piece.
Tokens of the Vanquished can also be exchanged for Season 2 PvP set pieces instead of PvE; visit Olus on the upper level of the Sun's Reach Armory on the Isle of Quel'Danas if you want the PvP versions, but note those pieces may not fulfill the set requirement for this achievement.
