The Chosen
Defeat Helya in Trial of Valor on Mythic difficulty without failing the Test of the Chosen.
The Chosen is a Feat of Strength earned during Legion by completing the Trial of Valor raid on Mythic difficulty without dying once across all three bosses in a single run, rewarding the title The Chosen.
Note: Achievement Unobtainable
The Chosen was made unobtainable when Patch 8.0 launched on July 17th, 2018, and was moved to the Feat of Strength category. It cannot be earned in the current game.
Understand the Buff Requirement
Upon entering Trial of Valor on Mythic difficulty, a buff called Test of the Chosen was applied to each player individually when Odyn began his roleplay or at the moment of his pull. Any player who died at any point during the run lost this buff permanently for that lockout.
Defeat All Three Bosses
With the Test of the Chosen buff active, players needed to defeat all three bosses in a single run without dying, with the achievement triggering upon the death of Helya. Because tracking was per-player, individuals who survived while others died still qualified.
Survival Mechanics Preserved Buff
If a raid wipe occurred but a player remained alive through abilities such as Vanish or Invisibility, that player retained the Test of the Chosen buff and could still earn the achievement on a subsequent pull within the same lockout.
Claim Testament of the Valorous
Completing the achievement caused a 
Testament of the Valorous to be mailed to the character that earned it, which was the only source of the Mythic Trial of Valor Helmets and Shoulders transmog ensemble pieces.
