Now You're Thinking With Portals is a 10-point exploration achievement earned by activating all teleporters located throughout Suramar.
Complete Oculeth's Workshop Quest
Before any teleporters can be activated, you must finish the quest chain starting with Oculeth's Workshop. All teleporters send you to Shal'aran, and the nearest flight point to the teleporter room hub is Meredil.
Activate Ruins of Elune'eth Teleporter
Find the teleporter at 36.2, 47.1 in Suramar — it is located directly above the teleporter room in Meredil.
Activate Falanaar Teleporter
Travel to the teleporter at 21.5, 29.9 in Suramar; the entrance to the area is at 22.9, 35.8.
Activate The Waning Crescent Teleporter
Find the teleporter at 47.5, 82.0 in Suramar, corresponding to The Waning Crescent at 47.6, 81.6.
Activate Felsoul Hold Teleporter
Head to the teleporter at 39.1, 76.3 in Suramar; the entrance to the area is at 35.8, 82.1.
Activate Lunastre Estate Teleporter
Find the teleporter at 43.6, 79.1 in Suramar.
Activate Moon Guard Stronghold Teleporter
Travel to the teleporter at 30.8, 10.9 in Suramar; the entrance to the area is at 27.8, 22.3.
Activate Sanctum of Order Teleporter
Find the teleporter at 43.4, 60.7 in Suramar; the entrance to this area is at 46.0, 64.6.
Activate Tel'anor Teleporter
Find the teleporter at 42.2, 35.4 in Suramar.
Activate Twilight Vineyards Teleporter
Find the teleporter at 64.0, 60.4 in Suramar; this teleporter requires Honored reputation and is unlocked after completing the Vineyards quest chain ending with Vengeance for Margaux.
Activate Astravar Harbor Teleporter
Find the teleporter at 54.4, 69.5 in Suramar.

The teleporter room in Meredil serves as the central hub — activating each teleporter in the field unlocks its corresponding portal there, from left to right: Felsoul Hold, Falanaar, Moon Guard Stronghold, Lunastre Estate, Ruins of Elune'eth, The Waning Crescent, Sanctum of Order, Tel'anor, Twilight Vineyards, and Astravar Harbor.
